Overview
The worksheet is the point-of-data entry in the system. Topics And Criterion establish the indicators that you would want to review, and are reviewed here in a worksheet. If you wanted to monitor an indicator for all the discharges that took place in your facility, then the number of worksheets for that Review Template will be equal to total discharges. However, typically worksheet data is monitored for a much smaller sample size. How big should the sample size be will depend on your own internal policies. Report on how many worksheets have been review can be generated from Number Of Criterion Evaluated.
Worksheet data entry can be performed:
▪On the workstation directly as the review is being done by the reviewer.
▪Using a hard copy and by entering data by a data entry operator.
Data entry user and last update date information can be captured automatically at the worksheet header or criterion level by defining the User and Last Update date fields at the header or criterion level respectively.
Each worksheet is uniquely identified by its primary fields; learn more about them at Worksheet Fields. Data entered for a Worksheet in one Review Template can be made available in another one if two Review Template share the same Topic.
Components
Header Fields: Worksheet Fields which do not change for individual Criterion are defined as Header Fields. They are typically demograpic information e.g. Account Number, Patient Name, Admit Date. Facility is a mandatory Worksheet Header Field. This is the only field which cannot be changed, once data entry for a worksheet has started. Worksheet Fields which are not defined as Review Specific; changing them in one Review Template would also change them in other Review Templates.
Criterion: Actual monitoring of the indicators is done at the Criterion. If a Topic or Criterion needs to be reviewed more than once for the same worksheet it can be repeated using the 'Repeat Criterion' feature. This will also allow you to assign different criterion attributes (e.g. Physicians) for different episodes.
Criterion Fields: Worksheet Fields which can change for each Criterion are defined as Criterion Fields. You cannot repeat a Criterion Field for a Criterion e.g. there cannot be 2 practitioners defined for the same Criterion.
Criterion Comments: You have the option to apply comments to specific Criterion. These comments will appear when you print the worksheet and in General Compliance Report. Comment can be a maximum of 8000 characters long.
